Default Need help creating form that will display query data.

I have a table that contain all us zip codes - each zip code has eight
additional fields of data that is specific to my business. I want to create a
form that will allow users to enter a zip code and in the form all nine
fields associated with that specific zip code will display. I don't know
where to begin.

My data look like this

Zip State City Zone ID Range
12345 PR Wemb West ETR-98 Bn - Gk
