Default HELP!!!

I have been trying all week to figure out how to link different fields in my
form. What I ultimately want to do is select a name from the first drop down
list and have the other two fields be automatically filled in with the
correct data. I know it has something to do with linking or binding but I
just started working in this program and I haven't figured it out yet.

Can someone please help me???!!!
