Default How to add multiple rows from data source into single merge doc?

I have an excel worksheet with multiple rows of different data for
individuals. How can get this information to merge into a table in the merge
document template?

example
John Smith - Student Name#1 - Program - Semester
John Smith - Student Name#2 - Program - Semester
John Smith - Student Name#3 - Program - Semester
