Default How do I create fill in fields within a document?

I am creating a document for my office and I would like to have most of the
text to be locked as it is "boiler plate" language. I would, however, like
certain fields to be editable so anyone may fill in case specific information
into certain fields (i.e. photographs, lists, paragraphs).
