Default Inputting only needed information

I would like to have a "front page" for inputting lease information
(Landlord, Tenant, Address (could this be tied to a square footage table?),
etc.). Ideally, this form would look like a brief form for agents to fill
out, and after inputting would generate the entire lease document. Where do
I get started?
