Default Problem formatting bullets and numbering

I am a medical transcriptionist and I need to make a list that looks like this:

A: 1. Over-read of the chest x-ray done 08/22/05 demonstrates a left

hilar fullness. Follow up recommended.

2. Smoker.

3. Dyslipidemia.

If I type A: and then hit the numbering button it puts a 1. in front of the
A:. I can change the indent and type the numbers manually, but then the text
lines revert back to the tab instead of being justified with each other.
Someone please help!!! I'm about to lose my mind trying to figure this out!!!

I would set it up in a table, Tim. Put the A in the first column, and the
numbered items in the 2nd column. Or just decide that A can't be on the same
line as 1.
