Default how do I make a voting ballot?

Any easy ways to make a ballot with six candidates, space for write in as well.

Default how do I make a voting ballot?

This should not be a difficult thing to do. A couple of squares to check,
the candidates' names or a blank line. Am I missing something?

The harder part should be deciding who deserves your vote. Is "None of the
above" a valid option? ;-)
