Default Insert a checkable box

Right click on any visible tool bar and select either the FORMS toolbar or
the CONTROL TOOLBOX tool bar. Either of these toolbars has a clickable check
box available. If you only want to check the box on/off, with no underlying
coding triggered by the click I would suggest using the FORMS toolbar
checkbox.
