Default footnote numbering messed up

The footnotes in my documents, which should be
automatically numbered, are missing random numbers (8,
10, 15-17, 26)...any suggestions on what went wrong and
how to fix it?

If you're tracking changes, you will probably need to Accept All Changes
before you get this straightened out.
