Default automatically building a document in word

I want to create a template for surgical procedures. I
want the template to have buttons that I can press and
the appropriate file automatically inserted into the
text. Multiple files have to be inserted into the final
document so that various variations to the operative
procedure can be used. The buttons need to be invisible
in the printed text. Is this possible to do in word 2003
or is there another product that this can be done in and
then converted to word?
