Default surya and siva

hi i have a senario, please give me formula
A B c d
site patient querystatus querycount
209 517 answered 5
209 517 opened 2
209 517 candidate 1
209 731 opened 5
210 734 opened 6
210 735 candidate 7
210 735 opened 4
another sheet
site patient issues count querystatus
209 517 8 answered 5; opened 2; candidat 1

209 731 5 opened 5
210 734 6 opened 6
210 735 11 candidate 7; opened 4

I want in a particular site (it varies) eg: in 209 (A) site, under patient
(B), based on the querystatus (c) category that is (Answered, Opened,
Candidate) the query count should come in another sheet in column c, and
querystatus of that pateint in column d in single cell, like in 209 site for
patient 517 there are Answered, opened, candidate queries, so they should
appear in a single cell as "Answered 5; opened 2; candidate 1.
imp..this should only happaen when site and patient id's are same. Its a
kind of consolidation. please revert me soon.
