Default Retain Data Off Word 2003 Page

I am looking into ways for our client to enter data via a VERY simple form
(NO back-end).
If I could have them access a few drop downs, check boxes, radio buttons off
of a Word document, and have that data saved to a comma delimited file that
would be great.
Sort of like a screen scrape of the data selected off a word document.
I think I could do via Frontpage but I'd like to try it off word.
Any ideas...

How about using formfields and the Save data only for forms option.

Hope this helps.

Yea, that is what I am looking into now. Trying to run the macro to save as
the *.txt , I know our clients will balk at the file save as piece...
Also, I do not see a way to connect (ODBC) the form to a backend. Do you
know if that is possible.
Mabye a simple html form with odbc may be a better answer...
