Default forms on contacts

J have made a form to act as page 2 on my contacts but I am unable to make it
appear when I open any contact as a default. If I can do this then I then
will be able to fill out the form for each contact separately - I don't want
to import anything into the form just to come up with the default sio I can
fill it in for each contact. Can a nyone help me with this please
I have business contact manager and could use this if necessary
