Default caption numbering

With Word 2007, on several documents I have numbered Heading 1 with
the "Chapter X" numbering,
so that my table captions will be numbered: Table 3.1, Table 3.2
etc.

Often when I save the file and reopen it the Heading numbering
disappears and the captions go back to Table 0.1, 0.2....

What did I do wrong?

Are you sure that heading numbering is correctly defined? Place the
insertion point in a Heading 1. On the Home tab, click Multilevel List, and
choose Define New Multilevel List. Then see

Default caption numbering

Also make sure that there are no blank paragraphs where the heading numbering
has been suppressed, since this would definitely make the first number in a
caption display zero (because it tries to reference a non-existing paragraph
number).
