Seriously, though, my suggestion is to use Word. It's "Table" feature is
probably the most direct approach.

In the first line of a new Word document, put the title, maybe just
"Routing Slip."

Then, put the cursor after the title and use
Table Insert table
to get a table with the number of rows and columns you need.

Then fill in the table with the information needed for the Routing Slip.

Add more text in or around the table to suit your needs.
