Default Templates and forms

I need help with creating a form as a template. Some one has presented me with a .dot document that lets you fill in little windows and when you complete these boxes, it shows the final document complete with the info from the little windows. I am not an expert in word, and would sure like so advice to what I need to learn to create these documents. Thank you in advance. Jennifer

Jennifer, take a look at the series of tutorials by Word MVP Dian D.
Chapman. Follow this link: http://word.mvps.org/tutorials/FreeToAccess.htm,
then scroll down the page to "Please Fill Out This Form!" Part 1 will get
you off to a good start.

That sounds like a userform.
