Default how to create a 2 sided document in publisher?

Can someone tell me if i am just missing this... or how to create a 2-sided
document in publisher 2003.

Exactly the same way as you would create a single-sided document, except
that you print it on both sides of the sheet, and perhaps set up
mirrored margins using Arrange Layout Guides.

How the document is printed is a function of your printer driver; many
printers can be set up to print both sides of the paper automatically -
take a look in File Page Setup Printer and Paper Properties.

Insert
Page
