Default how do i creat something like this?

hey guys i need to create something that looks like the picture below, i made
it in ms paint because i need to know how to get it to work in either access
or excell, does anyone know how i get say 1000 or so contacts into the fields
i have written and be able to cycle threw one by one using the next button,
and links to guides on how to creat such a databae is truely welcome. PS this
is for a call centre so my call agents can just keep goin threw each person
one by one, thanks, i also forgot to add the contact number into the picture,
thanks

Ok i have worked out how to design the database in access, but what i need to
know is how do i get the data into the fields, this is a rough one i have
done http://i45.tinypic.com/xp36e0.png i need to get the data from "sheet 1"
which has a list of names and numbers, into the fields in "form 1", thanks

Default how do i creat something like this?

Nathanmcc -

If your fields in your table1 match those in your sheet1, then you can
select the whole sheet1 (like you do in excel - the top-left corner of the
datasheet view), and paste it into table1. You can move the columns around
to make them match up.

Note - it will be much easier to maintain if you give your tables and field
meaningful names (table1 doesn't mean anything to me).
