Default Check boxes in word - Help!

Hi i am creating a project request form for people to fill out on their computer. It consists of yes and no checkboxes and then other areas where the user needs to type in information. I have reviewed all of the posts but don't find anything that will help me here. If i lock the template it works great except the user can fill in the other areas. When i don't lock it everytime they clieck the box an Check box menu comes up. Anyone help????

Use a locked on-line form and either add text fields or insert an
unprotected section for free text.

thanks for your help, but i don't want to use an on-line form, just regular Word. Tried to protect certain area's and not others but then the check box "menu" still comes up. Maybe i'll just go back to Excel.....err!

There are a number of ways to handle checkboxes in documents / templates.
Take a look at the Checkbox template available at URL:
http://addbalance.com/word/download.htm for an exploration of these.
