Default receipts/templates/forms

I need help with a sales receipt /contract between client and myself for a
series of treatments I will provide to client for agreed upon price per
visit and how many visits for the price. also a disclaimer/ no refund policy.
I am a hands on person but admitt to knowing absolutely nothing at all about
contracts or ways of protecting my assets. can anybody help me?

Are you asking for legal advice, or for advice designing an onscreen
Form in an Access database? (The latter is the subject of this
newsgroup). If you want legal advice about the wording of a contract,
I'd consult a local legal professional, one who knows the laws of your
jurisdiction.
