Default 12 entry of 3rd series is not plotted

In my simple bar graph I have 3 series with 12 entries each, however, Excel
2003 is not accepting the 12 entry of the 3rd series of data, even though the
first 2 series have 12 entries in each of them.
Any clues?

What do you mean by "not accepting"?

You should have four columns of data and 13 rows. The top left cell should
be blank, the rest of the top row should contain labels for the three data
series, the first column (save the blank cell) should contain category
labels, and the rest of the range should contain 12 Y values for each of the
three series. Select this entire data range and create your chart.
