Trying to add my name to the report header

I need to add my name to the top of my report header but i'm not entering in
the correct "code"

If you are posting this message via the Microsoft Office Discussion Groups
website, you should select the "Question" option under New, rather then the
"Suggestion for Microsoft".

But to address your question.

Where do you want your name, actually in the header?

What "code" are you using now, if any, that has not worked?

Are you the only one who will be using these reports? If so, just add a
label and put your name in the label.
