Default Create reports with PDF fields?

I keep track of over 400 clubs and their member information. I have created
a report where I can input the club's unique number to generate that
particular club's member information. I convert these reports to PDF and
attach to an email for the club's review and edits. Currently, they make
these edits to their reports by hand and mail the report back to me. Is it
possible to create PDF fields into the report to allow the club to make
corrections to name, addresses, phone number, etc. directly into the report?
I don't want them to change the database info in the report. I would like to
have the interactive field below the generated info. I can do this with
Adobe Acrobat, but I would have to place and arrange approximately 150 fields
per each report saved in PDF for a total of 60,00 PDF fields. Thanks for
your help.
