Default How do I customize fields in a "desin form"

I am trying to design my own contact form but am unsure how to do it. I want
to remove some ifields that are there by default, and add some fields of my
own. For example, I am trying to create a form for family members,, so I'd
like to remove business phone # and replace with cell #, and remove company
name, and replace with spouse.
Any helf is greatly appreciated.
