Auto Insertion contact name, company, tel, fax & email from Outlook

Default Auto Insertion contact name, company, tel, fax & email from Outlook

Application: Microsoft word 2000 Word template form ,
automatic insertion of Outlook (2000 SP3)address book
information, contact name, company, tel,fax & email

Goal:
Please refer to the below template form boxes.
What I would like to do is the following:
1 Place pointer to the right of the contact name
form field
2 click a button somewhere in the word document
which would excite my MS Outlook address book.
3 From the address book search for the desired
contact.
4 When desired contact is located click the contact
name
After clicking the contact name the contacts name
should automatically be written to the below contact field
Along with their Company name, Tel, Fax & Email
address

Word template form example:
Customer Ref:
Contact: Mr. Ms
Company:
Tel
Fax
E mail *

If anyone has any ideas as to how I may achieve this
please let me know
(PS have tried insert address button but its not quite
what I require)

Would be grateful for any ideas
