Optional "continued" headings?

I am formatting text that extends over several pages. At the top of
the first page of the section, I want it to say

Annex C - List of Facilities

and then, on the second and subsequent pages:

Annex C - List of Facilities (cont.)

The text after this heading can grow or shrink. If the text shrinks
to just one page, I have to delete this "continued" heading. Or if it
grows, I have to add the new heading at the appropriate locations in
the text, or move it up and down as the text is modified.

What I am trying to accomplish it to make the document "robust", so
that I don't have to do add the "continued" version of this page-top
heading at the correct locations every time I change the text in this
section..

(I'm trying to avoid adding more section breaks to the document, or
more items to an already crowded Header and Footer.)

Another way that it might be implemented: Word lets us define
"header" rows that appear at the top of the page when a table extends
over more than one page. What would be cool is to let us add a
"header row" that would appear ONLY if the table extends to a second
page. If the table is shorter than that, this header row would be
invisible.

Any thoughts?

Using section breaks and the "Different first page" option to create a
separate header (and footer) for the first page will be the easiest
solution.
