Default Template Fill-in Pop up box


Help, I have created a template for corporate-wide use
that has an irrelvant and anoying pop-up fill-in box that
apprears when this template is used as a document.
Each time it is opened, printed or saved this pop-up
appears for EACH page of the document.

Obviously this dosen't work. I do not know what or where
it is or how to remove.

Please advise on locating and removal. Thanks!

Default Template Fill-in Pop up box

Toggle on the field codes with Alt+F9 and you should be able to locate it.
