Default Add a Fill-In Form to a web page

How do I put a form on a web site (not a SharePoint site), and be able to
allow people to put in data?

This is a class evaluation form, and I want the data to go directly into my
Access database. But when I put the form on a test web site, the drop-down
selections don't work.

Help.
