Outlook Form Loading errors

Default Outlook Form Loading errors

I am experiencing two problems that I believe are caused
by the same issue.

When trying to load a Contact I occasionally receive an
Outlook message indicating that the program can not find
the correct Outlook Form and recommends that I contact my
Administrator.

I run a stand alone system, not using Exchange. I think
this problem may be due to the fact that once I loaded
the Small Business portion of Outlook and I don't know to
change or make all Contacts use the same Form. Presently
on Outlook 2003, I do not use the Small Business package
that came with the product.

The two messages I receive pop up as dialog boxes and the
first reads:

The Custom Form could not be opened. Outlook will use an
Outlook from instead. The form required to view this
message cannot be displayed.
Contact your administrator.

The second message appeared when trying to create a New
contact from the Action drop down New Contact from Same
Company. A Microsoft Office Outlook dialog box popped up
with the following message:

The form required to view this message cannot be
displayed. Contact your administrator.
