Default How do I assign specific columns to a check box in Word

I'm creating a fill in form in Word, but I only need certain rows to be
filled out when they click one check box and other rows when they click the
other check box. How do I got about doing that in Word?

What type of form are you using? One with formfields in a document that is
protected? or a custom dialog (a UserForm)?
