Default BCM2003 Account form problem

I'm currently having a problem with BCM2003 when I select the contacts tab and then select the Accounts listing. When I click on an existing account, I receive the following message: "The custom form could not be opened. Outlook will use an Outlook form instead. The form required to view this message cannot be displayed. COntact your administrator." At this point, the standard Outlok 2003 contact form appears populated with the account info. Unfortunately, it doesn't allow for the same usefullness of tracking the email, contacts, tasks, etc. that the Account form allows.

I've already performed the following:
1. Saved/backed up the msbcm file for future use.
2. Saved the Outlook .pst file
3. Reinstalled BCM 2003.

My question: How do I reinstall/locate this custom Account form? Do I need to reinstall Outlook or Office 2003?

Any help would be greatly appreciated.

BCM sometimes has what's known as a "forms cache" problem. Clearing the
cache usually fixes it. Choose Tools | Options | Other | Advanced Options |
Custom Forms, then click Manage Forms, then Clear Cache. If simply clearing
the cache doesn't fix the issue, try the other steps detailed at
