Default request form using a userform?

I am creating a form which my co-workers will have a link to. They will
complete the form, click a submit button and the form will email to me. I'm
not very familiar with userforms in word but will this work for me?

Yes. You can do that, but I suspect you want what Word calls an "online
form." Check this in help. For more about online forms, follow the links at

Chapman's series of articles. You may also want to look at
