Default Printing Postcards

I'm trying to print postcards. I would like to print appointment information
on one side and the name and address on the other side.

My printer handles duplex printing. The cardstock we are using is 8.5 x 11
perforated with 3 postcards on each page.

How can I print 3 postcards on a page and then using the duplexing, print
the address on the reverse side?
