Hi rackman.

I have just put up a fully worked example for you, with step by step
instructions, at:-

It is the first item on the home page.

There is one small error in my file: I jumped from step number 10 to 12. I
missed out step number 11 but that was just a numbering issue; I didn't miss
out any instructions.

I have tested the file and it works so hope that it does the same for you!
