Open MS Word.. go to HELP on the top bar.. see where it says Microsoft
Office Online?.. click on it..

When the website opens, look to the left of the screen for the subheading
'Templates'..

No surprise really.. a suggestion card is not exactly difficult to make,
eh.. a card, not unlike a postcard, with the heading 'Suggestion' would
probably cut it..

I've got a simple one which I produced for where I work - prints as
double-sided A4, which can be cut down to A5. If you want to see it, I can
e-mail it to you.

Regards,
Julie
