Default Start by having a search form - HELP!!!

Good day! I am having a problem with deciding what to do for a search form. I
need to search a database for a text field and see if its in the database. If
so it needs to load that selectiojn to be viewed on the MAIN FORM. If not
there i need the MAIN FORM to open and be blank so that the data can be
entered. So basically I would like a form to pop up first over the MAIN FORM
with a search field and then the user can enter in the criteria to be
searched.
