Are there any templates available to design a "coupon book"? I need to make a
perforated book that our monthly sponsors could use to send in with support.
Any suggestions?

I've never seen any but that doesn't mean they don't exist. Did you try
Google?

Do you have any thought in mind on how large each coupon will be? Will you be
doing the perforation yourself? (Fiskars has a tool)

Do a booklet setup, portrait, custom size, type 4.25 width, 3.66 height. This
will give you three booklets per page.

No I won't do the perforating. I'd like to just design it and have sent to a
printer. Would it be best to just do the measurements I want and go from
there?

You first step should be to talk to your printer and find out what he/she
can handle.
