How do I get the help system to search offline by default
and only search the online help system if I choose to? I
want to do this even though I am online most of the
time.

Outlook, PPT, etc... Help/ click "Online Content Settings"
De-select any online option from there...

Office 2003:
-----------
Click the "Help" menu
Click "Customer Feedback Options"
Click the "Online Content" category
Uncheck "Search online content when connected"
Hit the "OK" button...
