Default Word Template with check boxes and option buttons

I need to create a word template that is formatted and
includes text boxes that will be entered by the end user
with option button and check boxes throughout and it will
tab through the entire document. I am not sure how to do
this. I have an existing document that I need to fix.
Any help would be greatly appreciated.

What you are talking about is what Word calls an "online form." Check this
in help. For more about online forms, follow the links at

Chapman's series of articles. Read the entire series because the option
button business can get a bit tricky.
