Default Creating Medical Forms

I work in the healthcare industry and we create our own forms using Word
mostly for medical forms department. Some of the forms can be very
complicated and may require areas where the end user inputs the information
directly into the form. We also protect these forms so the cannot be changed
by the end user unless areas are specified for input. We also have forms
that may require information to change, for example if the patient does not
need specific care in one area of the form it could filter so that area does
not appear on the form.

Does anyone know of any software that will help in this area or are there
tricks to Word?
