Default Need To Track Advertising

I am looking for either an Excel Ttemplate or a formula that will do the
following:

1. Enter various methods of advertising, ie, print, online, radio etc.
2. Enter cost associated with each marketing area.
3. Enter number of calls generated from each advertising source.
4. Enter dollar amount of sales relative to each ad source.

After above inputs the the spreadsheet needs to do the following

1. Return a number showing the following; ad a generated x calls, ad b
generated y calls, ad c generated w calls, ad a generated x percentage of
calls relative to all calls generated for all ads, ad b generated y
percentage of calls relative to all calls generated, etc..
2. Return a number that shows which ad is generating the most revenue
relative to all ad money spent.
3. Return a number that shows, as a percentage, the cost of the ad relative
to the moneys generated. ie ad a costs $10,000.00 to run, total sales
generated from add was $45000.00.
