Default Form Disapeard / deleted

I have a button on the startup menu that opens the "Dispatch Log" which opens
the repair history open only list. For some reason, we are getting the
following message: Record is deleted.

I do not have the option to view the form anymore. The only thing I see is
the startup form. I cannot even choose the forms/tables/queries list that I
used to see under the Window drop down box. Why would the record be deleted?
Is there something wrong with the macro on the button? How can I veiw the
forms list(i have about 4 different forms total)?

Hi, Breezy.
Close out the database then reopen, but hold down the 'Shift' key before you
doubleclick . When you hold down the shift key on your keyboard, you bypass
all the startup options you chose and get to see the actual database window,
forms, everything you created.
Maybe you can then figure out why you're receiving that "record is deleted"
message.
Good luck,
Simone
