Default missing radio button on Insert Address Dialog - inserting outlook contacts into Word

Any idea on how to fix missing dialog buttons when you are in Word and
insert an address from the contacts in Outlook??? I can only insert the
main mailing address, which works well enough, but I would like to insert
other addresses, such as the home address occasionally. I am using XP/SP1
and Office 2003.

A similar problem with the missing radio buttons in the Insert Address
dialog box (named "Select Name") in Word had previously been noted with
Outlook 2000: OL2000: (CW) Option Buttons Missing Accessing the Outlook
Address Book from Word Article ID: 240629

However, when I replaced the supposedly missing file: schdmapi.dll and run
the repair, it doesn't correct the problem.

Has anyone else noted this problem with XP and Office 2003? I didn't have
this problem with Office 2000.

Any ideas on how to fix it???

Explain what you mean. It is not clear to what dialog you are referring, not
what buttons you think are missing.
The Insert Address Dialog has always inserted the Mailing Address. There are
no other options.

In my version of Word 2000, when the insert address dialog came up, it had:

1. "Type name or select from list:" A place to choose the name.
2. "Show Names from the:" A place to choose either contacts or a PAB, if
you had one and most importantly for me

3. Three radio buttons: Mailing, Home and Other. You could check this and
choose which one was inserted.

This link has the exact picture of what my dialog looked like:

So what is your question?
You said you were asking about Word and Outlook 2003.
I answered.
Did you have another question? Are you asking whether there have been
changes in the currently supported versions? That answer is yes.
