Default Simple data entry form

I work for a retailer that makes me go out and inspect their stores every
week. When I do they make me use an excel spreadsheet that has like 50
questions that I must tab down, or arrow up/down to type in.
My question is: Is there a ready to use template that I can download or
devise to enter my 50 questions and then allow users to open the program and
fill in a form: "fill in the data and then hit return, enter date then hit
return etc etc" and then let it populate a simple to print spreadsheet? I
entered a question in the Excel forum but I would like to use a simple Access
Database. Please help if you have a few minutes. Really appreciate this
forum and the good people that answer these questions. Take care and thank
