What you are talking about is what Word calls an "online form." Check this in help. For more about online forms, follow the links at http://addbalance.com/word/wordwebresources.htm#Forms. One of them will take you to a KB article on how to do this. Follow up by reading Dian Chapman's series of articles and become an expert on form construction and use!
