Default Indexing Problems

I am using Windows XP SP2 and Office 2003 SP1. I have a lengthy document
100+ pages long of text that I want to index. I have created a
concordance file and stripped all formatting from it and done everything
possible to make it as generic as I can. The problem is that no matter
what I do the index is created with very strange formatting.

For example:

Some words are in all caps
Some words are underlined
Some words are bold
Some words are bold all caps
Even though I am using the index style some words have a different font
and point size.
The concordance file will have the following entry: accessory use
It will appear in the index on three lines as follows:
accessory use
Accessory use
Accessory Use

What am I doing wrong? Thanks for the help.

G'day ,

If you email the doc I can check it out for you, but there are several
likely culprits. One is the Index switches in use and the other is
character formatting around the actual XE field (View Hidden Text to
see them) and / or the presence of MERGEFORMAT.

The text is 1252k and the concordance file is 420k. The zip
is 336k.
I checked the word document after I ran the index function
and found all kinds of formatting inside the {xe."text"}.
For some reason Word is creating formatting for some but
not all the instances of a indexed word. I will send them
as zipped files. Thank you again.
