Default Editing ppt created in 2003

The placeholders must be on the master slides and then they appear (or
should appear) on all the slide layouts (the small thumbnails) associated
with those masters. If they don't, then turn them on on the ribbon or paste
them onto the layout from the slide master.

Then in Normal editing View, go to Insert | Header and Footer. Type in the
info you want to have appear and click Apply to All.
