Default ruler has incorrect measurements, what am I doing wrong?

I think most everyone here assumed you were posting the photo as a joke;
however if you are serious with your question --

The point was, what you see on the monitor has nothing to do with what
prints out, if you had a larger monitor, the ruler would appear larger
(perhaps even correctly scaled), if you had a very small monitor, the ruler
would be way out of scale. Publisher doesn't know what size monitor you are
using and adjust/calibrate the screen accordingly. Suppose, the image was
sent to a projector, as you moved the projector closer or further from the
wall, the size of the ruler will change drastically.

The ruler is there to show you what the printed output looks like.
