As far as I understand yr problem, you need to define a second level for the A,B, etc (heading 4). I understand you have already H1 (for the 7 in yr example) and H2 (for the .2 in yr example). Go into outline numbering for level 4 (customisation) and change 1.1.1.1 (the grey fields) into 1.1.1A . Make sure this "A" is a grey field so Word treats it as a field.
