This is a forum for Visio, so one of the Word forums should be able to help
you further.

Yes, this can easily be set up in Word. Since you are only talking about one
item, you really do not need a table, but the table will allow you to align
things.
This can be set up asa form so that everything except the quantity is
protected and you can add a calculation field to get a total.
